Optus reveals three people died after a tech failure affected triple-0 calls


Devastating details have come to light about the horrendous network upgrade failure from Optus that left three people dead.

September 20, 2025 - 6:44AM



One of the three people who died following a botched Optus network upgrade was a newborn baby.

The upgrade resulted in a technical failure impacting triple-0 calls in South Australia, the Northern Territory and Western Australia.

Optus chief executive Stephen Rue told reporters on Friday evening that two people died in SA and one in WA.

Overnight, SA Police identified the two people in their state, with a newborn among the dead.

“Yesterday, Optus conducted a network upgrade and within this process a technical failure impacted triple-0 calls,” Mr Rue said.

“This resulted in the failure of a number of triple-0 calls ... our investigation is ongoing but at this stage I can confirm that approximately 600 customers were potentially impacted, of which a proportion of their calls did not go through.

“I can confirm that this technical failure has now been rectified.

“I have been advised that during the process of conducting welfare checks, three of the triple-0 calls involved households where a person tragically passed away.”

Yes, the 000 calls failure should never have happened.

Having said that, it's a pity everything except the 000 service didn't go down for a few hours or even a whole day.

Watching the mindless sheep who have their faces stuck in their phones seemingly 24/7 having a meltdown because they can't use social media for even a short time always gives me a good laugh.

It's crazy.
For the Optus system to reject a call it had to
recognise that there was a 000 call.
It doesn't make sense that anyone would
program it like that.

It sounds like there was no engineering oversight with
the job ticked off as all OK – approved.
Someone just did it – cowboys.

I think it's time to change the culture of high tech companies.
No one should eligible to be the CEO of a company like Optus unless
they are a competent communications engineer -
so that they fully understand the technical side of the business and
can therefore ask the right questions to all the engineers.

Instead they put glorified accountants and MBA degree people in charge
who only understand money.
