The issue is one of principle - the Palestinians essentially want Israel to acknowledge that people were kicked out of their homes unjustly and to acknowledge an 'in-principle' only right of return for those families. In practice this might result in just a symbolic statement acknowledging this in-principle right, or a very small tokenistic acceptance of some refugees back to Israel. Certainly not demographic-breaking stuff. It should be understandable why Israel's refusal to even consider such a negotiation is a pretty major affront to the Palestinians.
