from my understanding of how it's supposed to work, he has a rough idea of where abouts on the screen the map will be, the rest is an educated guess based on experience.
The issue I have is that someone then has to digitally superimpose the map onto the footage ..... why don't they move it a bit higher or lower to avoid it looking like he's having an erection when they do that?

The presenter views a monitor displaying the chroma-key inserted image (the weather map) and themselves so they can point at the correct parts of the image and position themselves correctly.
