Equipment failure features very highly in fatalities, and if you've been a keen viewer of the aircrash documentaries on TV you'll know that cutting corners and slack practices by maintenance staff is a common occurrence.
The greatest factor militating against the maintenance of safe equipment is chatting between the employees. As a one-time inspector myself I put banter and boredom-busting chatter between workers as easily first place on the list that leads to faulty equipment and jobs not done properly.
In sensitive areas where safety is imperative it's important not to employ conversational egotists, immature personalities, and
wogs people whose ethnic culture demands an almost constant stream of backwards and forwards 'baby-chatter'.
Filipinos, Yugoslavs, Greeks, Indians, Italians, and such.
