corio bay clinic.  A local doctor says federal government cutbacks forced his clinic to abandon bulk billing.

Northern Bay Health introduced has switched from bulk billing to mixed billing, meaning it now only offers free treatment to children and concession card holders.

Some staff and patients were visibly distress when the changes came into effect in the last 24-hours.

The clinic's Dr Mark Kennedy said he hated the fact that adult patients would now be charged.

He said said that after nearly 30 years, their hand has finally been forced by the Morrison government.

"I have withstood every cut and every freeze up until now," Dr Kennedy said.

"But we can no longer keep providing a service when the government's actually cutting the fees."

Dr Kennedy believed most patients at the centre would still be bulk billed as they were either children, elderly or other concession card holders.

However he believed Northern Bay Health would not be the last practice to have to switch to mixed billing services.

Medicare bulk billing is dying in Tasmania and is unlikely to ever recover, according to a Mowbray GP who says the community must "reluctantly" get used to it.

Dr Andrew Jackson, of the Northern Suburbs Medical Service, says "abysmal" support from the federal government meant patients searching for GPs who bulk-bill for all their services, would increasingly do so in vain.   :(

One of only two medical practices servicing Launceston northern suburbs, the comments come after news Mowbray Medical is turning away more than 20 patients a day and is no longer able to offer mixed billing, as the result of what it claims has been a "persistent lack of government support".

Bass Liberal MHR Bridget Archer says she is committed to working with GPs in the region, adding she is not satisfied with accepting the situation was final.

A proposed visit by federal Health Minister Greg Hunt is expected to take place within the next two months.

However, Dr Jackson, who is a practice principal and full-time GP at the Mowbray clinic, said history had proven the government's failure on addressing the issue.

"There was no longer any hope that a federal government health minister would ride in on his or her white stallion and save the day for general practice by substantially increasing the dollar value of GP Medicare rebates - history has proven this," he said.

Echoing comments made by Australian Medical Association president Dr Tony Bartone earlier this year, Dr Jackson said Medicare rebates for GP services would be around two-and-a-half times higher than the level if they had been fairly and properly indexed.

Instead of the standard bulk-bill rate being $38, Dr Jackson said it would closer to about $95.

At this level, Dr Jackson suggested most GPs would be prepared to bulk-bill nearly all of their services, or instead charge a relatively small "gap" payment for those who could afford it.

"For GPs who want to practice highly quality medicine, not turn over patients at sausage-machine rates, invest in new medical equipment and training, and be able to retire from practice at a seemly age, the $38 rebate has lost any meaning and cannot be acceptable for anyone who wants to have a long-term career in general practice," he said.

Dr Jackson said his practice used bulk billing heavily in some important areas to support local community health, including the immunisation of more than 1000 patients for influenza.

"We will shortly be running a whooping cough immunisation campaign with bulk billing," he said.

"We bulk-bill diabetes assessments and mental health plans, among others.

"We also bulk-bill our new humanitarian refugees. And at our Newnham nursing home where we provide care for 65 frail elderly residents, we continue to bulk-bill them all."

Ms Archer said she was working to finalise the date of a proposed forum with Mr Hunt and others in the health field, as part of an upcoming visit to Tasmania expected to take place within the next six to eight weeks.

"I am committed to working with the GPs in Northern Tasmania and the [health] minister to constructively work on a range of issues that have been raised with me by medical practices in my community," she said.

"Bulk billing is certainly one of these issues and I am not satisfied with accepting that the current situation is final and I will continue to do what I can to look at creating effective change in this area."

Despite the challenges, Dr Jackson said it wasn't all "doom and gloom" for the area, with the Northern Suburbs Medical Service still accepting new patients - including those impacted by the closure of the Caledonian Medical Centre in April.

Dr Jackson said the closure of Invermay's only health centre and the "scattering" of its doctors elsewhere in Launceston, had been a blow to the northern suburbs.

"But more good news is that my practice advertised locally that Caledonian patients were welcome to attend our practice which they have, our books are not closed, and we are not turning anyone away," he said.

The practice has also been successful in recruiting additional GPs, with recent expansions also helping to take pressures off hospital emergency departments.

"Part of this development is a two-bed infusion bay to enable the giving of treatments that previously meant sending many patients to the hospital including for urgent care," he said.

"Being able to pay doctors well and have great facilities and staff attracts doctors to our practice and can help them stay in the long term - this is a good news story for our northern suburbs."

Screw that. It will only die if we sit and watch it happen while not doing anything about it.

We need to have an opposition with some spine, who will make this issue front and centre at the next election.

Save medicare, save bulk billing - I'll vote for that.

You'll need to control the doctor's Union first... can't go without first class tickets to Aspen twice a year .... business trip on tax, though .. gotta meet up with some colleagues and discuss ....

Put 'em all on state salary ....... just went through six years of increasing chest pain that often nearly brought me to tears... even the Learned Cardiologist kept wanting to repeat stress echo tests... same non-result every time but filled his coffers ... quack after quack shuffling it off as 'chest wall pain' or rib joint pain or gastric reflux ....into hospital - one hour in cardiac surgery and pain gone.... now I can bend down to wash feet and put on socks and shoes etc without suffering...

Where is their Hippocratic Oath these days?  In their wallet?  8-)

I agree with what's-'is-name below - blame Labor as well for not taking the balls up to the opposition... NONE of these bastards in politics give a damn unless it suits their ideology and their self-interest... Labor is far too busy sucking up to the women's vote to no avail and all the other divisive elements with stupid and endless demands...

Fat Joe's Freeze has killed off much bulk billing - which is all that many can afford... he needs to be exposed on a hillside in Winter in the snow himself.... his parents missed a golden opportunity there...

Only a bastard doctor would knowingly suck up every last available cent an unemployed person etc has, just to give simple treatment... how low can anyone get and still claim to be a doctor?

I prefer to travel over an hour each way to continue to see my GP occasionally - since she is a genuine caring doctor who works for her patients and doesn't charge double or more...  if they think it's all about them, they should never have been allowed into medical school ....

Put 'em all on salary ....

I've done a couple of Cardio Calcium Scoring tests and have scored zero both times.

Well, 0.01 - they say they can't actually give a score of zero.

The doctor wants to keep sending me for more though.

They can't be claimed on medicare, and they cost $170.

I suspect my doctor is getting a kickback for every test they recommend.

With a score of zero (0.01), there's no need for me to have another test just yet, and there's no need for me to take statins.

That doesn't stop the doctors wanting to prescribe them though.

Stress test plus consultation - $330 + $130 = $460 - but the patient only loses about $150 or so - how does someone unemployed get that together and eat?

Surgery in hospital - absolutely free .... one hour's work by surgeons.... and home next day ....

Every civilized country has some level of publicly funded health care.

When juliar has to resort to gofundme in a desperate bid to raise the money he needs for a life-saving bone marrow transplant (or whatever) - no longer provided or even subsidised by the state - lets see how much he cheers on the systematic dismantling of "socialism" - aka a viable public health system by his liberal fanboys then.

It pays to not get sick. Don't drink, don't smoke, don't touch drugs, don't over eat, avoid sugar, fat and salt and get plenty of exercise.

Gandy should note, there's always private health insurance and you are right as rain.

The AMA is the most militant union on earth.
They deliberately hold back doctor numbers so as to maintain a monopoly.

Doctors themselves are greedy, seriously greedy.
How much money do they want?

But When it comes down to it, doctors are simply mechanics, bad mechanics.
The body generally heals itself after a problem is removed.
Doctors woukd still be running around shaking rattles and wearing weird headgear if it weren't for the development of pharmaceutical compounds.

They are the greedy poor cousins of engineers, chemists and true professionals.
And it's time they were made to realise it.
